Kindly note that payment for services, including payment of co-pays, deductibles, or charges for out of network care, is due at the time of checkout, though payment plans may be available in certain circumstances. We accept cash, checks, or credit cards.

In the event that we are not in network with your plan or you are uninsured, Family Medical Health also offers reasonable prices for patients paying out-of-pocket (Note: Some plans prohibit members from paying cash for covered services.)
